Please make a check payable to MSD and mail to LOJIC/MSD, 700 W Liberty St, Louisville, KY 40203 along with completed form. Upon receipt, your order will be mailed to you.

Credit Card Message

Please email your order to LOJICSupport@louisvillemsd.org, making sure to provide your phone number. A customer relations person will contact you to process your payment. Upon verification of payment from customer relations, your order will be mailed to you.
