Instructions for Using the MSD Floodplain Determination Application
SYSTEM REQUIREMENTS:
Using the Application: To make a Floodplain Determination, you will need to know either the street address or parcel ID for the property in question. Click on the respective tab and fill in the required information listed below:
Personal Information: Enter the name, phone number and E-mail address of the person making the request. Company name and Fax Number are optional at this time. Manual determinations will be sent to you by E-mail, therefore submitting a valid E-mail address is imperative. Location Information: Either a valid address or parcel ID depending on which tab you selected. (See below for tips on entering location information.) Once the personal and location information is entered, click the Submit Request button to proceed. You may also click on the Clear button to clear the form to start over. During the processing of your request, the words "Processing..." should appear in the left margin showing that the application is processing your request. Several windows may appear on your screen and then disappear. Once the Processing is Complete: If a floodplain determination can be made for the property in question, a Floodplain Determination Form will be displayed on the screen in Adobe Acrobat format (a .pdf file). You can then print the form for your records. There are times when a floodplain determination cannot be made automatically and must be reviewed by MSD staff. In these cases the following message will appear.
Please record the Request Number provided for reference in case you need to call MSD and check the status of your request. Tips for Entering a Street Address: When entering a Street Address make sure you have typed in the house number, street direction (if one exists), street name, and street type in the space provided. Street information is not case sensitive. Do not type in an apartment number or any type of punctuation. Once the address has been entered, press the Submit Request button to proceed. For street direction and street type abbreviations, please see the tables below. Street Address Examples: 700 W Liberty St 700 w liberty st 700 west liberty street
For 9109 Raintree Drive, Apt 1 Enter 9109 Raintree Drive If the application finds a match with the address you entered, it will begin processing. If the application is unable to locate the address, it will display a list of possible choices. For example, when entering 2600 Colen Ave a list of valid street name choices will be displayed as follows. This is because the street name was spelled incorrectly:
Select the appropriate street name to continue. Next a list of possible addresses for the street name you selected will be displayed:
Select the appropriate address to continue. This address will be the one to appear on the Floodplain Determination form. If the incorrect house number was entered and the street name was spelled correctly, then the above 'Possible Street Addresses' list will be displayed asking you to select from the list of possible addresses for the street you entered.
Acceptable Abbreviations
Tips for Entering a Parcel ID: When entering a Parcel ID, enter the entire tax block, lot and sublot in the spaces provided. Parcel IDs can have letters in them or leading zeros. The application is not case sensitive and leading zeros do not need to be entered. Tax block and lot entries are required but the sublot number is not always used. If a sublot number is not used or is all zeros, the sublot field can be left blank.
Parcel ID Example: 014J00570000 Block field can be entered as 14j Lot field can be entered as 57 Sublot field can be left blank
Once the Parcel ID has been entered, press the Submit Request button. If the application finds a match with the Parcel ID you entered, it will begin processing. If the application is unable to locate the Parcel ID it will display a list of possible Parcel IDs that are similar to the one you entered.
